Fallout 76’s latest update, "The Ghoul Within," introduces an immersive way for players to experience life as a Ghoul in a radioactive world. As announced by Bethesda on March 18, Season 20 brings a host of ghoul-themed additions, from unique abilities and mechanics to fresh cosmetic items that let you customize your C.A.M.P.

To begin your transformation, complete the "Leap of Faith" questline, which unlocks the Ghoulification process. This journey takes you deep into a previously unexplored part of the Savage Divide, where you’ll meet new characters who guide you through becoming a Ghoul. Once transformed, Ghouls gain access to two exclusive abilities—Glow and Feral—and a total of 30 brand-new Ghoul-exclusive Perks.

The Feral ability replaces traditional hunger and thirst mechanics, offering dynamic stat boosts depending on the Feral meter’s fill level. However, letting it drop to 0% triggers extreme effects: +150% Melee Damage, but also severe penalties like -5 Endurance, -99 Charisma, -30 Max HP, -20 Max AP, and drastically reduced weapon accuracy.

The Glow ability allows Ghouls to absorb radiation instead of fearing it, granting immunity to diseases and mutations while literally making your character glow. It also increases maximum health and heals existing damage. Glow synergizes with the 28 Ghoul-exclusive perks (or "Gherks") and two new legendary perk cards introduced in this update.

Please note: traditional perks related to Hunger, Thirst, and Chem Resistance are unavailable to Ghouls since they no longer rely on those systems.

Embracing your Ghoul form makes navigating the irradiated wilderness more manageable thanks to enhanced survivability and powerful Gherks. However, not everyone in the wasteland is thrilled about your new look—factions like the Brotherhood of Steel will treat you as hostile, potentially locking you out of certain quests or interactions.

Thankfully, a new NPC named Jaye offers solutions. She provides disguises that let you interact with content otherwise blocked due to your Ghoul status. If being a Ghoul doesn’t suit you long-term, you can revert back to human via the Character Screen. Keep in mind, though, that this change is permanent—you won’t be able to retake the "Leap of Faith" quest. However, if you ever want to return to your Ghoul form, you can repurchase the Ghoul Retransformation option for 1000 Atoms.

Whether you're a new player or returning veteran, the Level 50 Character Boost helps you jump straight into the action. Priced at 1500 Atoms, this boost grants immediate access to key game features such as Daily Ops, Public Events, and major storylines—no grinding required.

The Season 20 patch also includes numerous bug fixes and balance adjustments covering accessibility improvements, combat tuning, and weapon damage tweaks. Bethesda has also teased the next major update, "The Big Bloom," scheduled for release on April 29, as outlined in their official season roadmap.

Originally launched in November 2018 with a turbulent reception due to bugs and performance issues, Fallout 76 has steadily improved through consistent updates. With Season 20 now live, the game holds a “Mostly Positive” rating on Steam, with 76% of user reviews being favorable. Fallout 76 is currently available on PlayStation 5, PlayStation 4, Xbox Series X|S, Xbox One, and PC platforms.
