This dark ending, recently rediscovered during testing of the eighth major patch, allows the protagonist to eliminate the illithid parasite by forcefully removing and destroying it, leaving the hero unharmed. A subsequent choice presents itself: depart with companions, or leave them behind and go solo.
Many believe this ending will be fully implemented with the release of Baldur's Gate 3's eighth patch.
